Summer Camp Registration & Waiver form

PLEASE READ CAREFULLY BEFORE SIGNING BELOW!

Swim Fanatics Summer Camp Policies, Rules, & Waiver: 

 • Swim Fanatics uses the Band App for all communication with those that are registered in any of our swim sessions. Once your registration has been received, you will receive a message with an invite to join the Swim Fanatics Band App. Please be sure to join this to ensure you receive clear and timely communication. The 336-287-6109 is for new inquiries and sales only. (Only registered members are able to view what is in the app).
•If inclement weather causes us to move indoors, we will notify you in the Band app of any changes and our alternate plans. (Parents will have the option to pick up child from camp, transport the child to an alternate location, or request transportation from staff or from another camper’s parent).
• If camp is cancelled due to weather, it will be posted on the Band App.
• If you need to cancel, please notify Camp Staff via chat in Band prior to the start of the camp day. (A private group chat between you and Camp Staff will be created once you join the Band App.)
•You can make up the cancelled or missed camp day on another day during another summer camp if there is space or you can trade out the missed day for a swim lesson at our Indoor Marriott Location or Lawndale Pool Location (Summer/Outdoor).
• Swim Fanatics does not give refunds for any reason.
• A make-up credit will be offered if the participant has a medical exemption letter stating they are not able to participate. This credit is not a refund and will be good for 1 full year towards additional programming.
• Swimmers must be picked up by 1 PM (Half-Day) or 5 pm (Full-Day). There is a 5 minute grace period but after that the parent will be charged $1 per minute that the swimmer remains at camp.
• Plastic water bottles only! Reusable or disposable bottles are accepted. We suggest bringing plenty of water for your camper. No sodas please.
• When you register for any Swim Fanatics Program, you are agreeing to all of these terms.
• A waiver (Electronic copy prior to camp) and (Paper copy on the 1st day of camp) must be signed before a swimmer is allowed to attend camp and swim.

THIS IS A RELEASE OF LIABILITY AND WAIVER OF CERTAIN LEGAL RIGHTS.

I (SIGN YOUR NAME BELOW) the parent/legal guardian of the participant agree and understand that swimming is a HAZARDOUS activity. 

I recognize that there are risks inherent in the sport of swimming, including but not limited to, paralyzing injuries and death.

I, the parent/legal guardian of the participant hereby agree to the Swim Fanatics Swim Club Summer Camp program contract and hereby agree to hold harmless Swim Fanatics Swim Club, its instructors, counselors, coaches, officers, directors, agents and employees against any liability resulting from any injury that may occur to the participant while participating in Swim Fanatics Swim Club Summer Camp program.

I, the parent/legal guardian of the participant also agree to indemnify Pinetop Sports Club/Friendly Acres Pool, its instructors, counselors, coaches, officers, directors, agents and employees for any damages incurred arising from any claims, demand, action or cause of action by the participant or their guest.

I, the parent/legal guardian of the participant also agrees to indemnify Greensboro-High Point Airport Marriott, Friendly Acres Pool, Pinetop Swim Club, The Lawndale Pool Swim Club for any damages incurred arising from any claims, demand, action or cause of action by the participant or their guest.

As the parent/legal guardian of the participant, I authorize any representative of Swim Fanatics Swim Club to have the participant or guest treated in any medical emergency during their time in Swim Fanatics Swim Club swim lesson program if I am not on the premises. 

Further, as the parent/legal guardian of the participant, you agree to pay all costs associated with medical care and transportation for the participant in the event of a medical emergency.

I, the parent/legal guardian of the participant have noted on the form below any medical/health problems the participant has, of which the staff should be aware.

I HAVE CAREFULLY READ THE ABOVE LIABILITY RELEASE AND SIGN BELOW WITH FULL KNOWLEDGE OF ITS CONTENTS AND SIGNIFICANCE.
